A Cargo Claim form should be completed if a shipment has been delivered, but content is missing or the shipment is thought to be lost. The claim must be submitted by the party that owns the claim rights (usually the shipper) otherwise written authorization from that party (termed a “release”) is required. Please review items required and gather them prior to completing the online claim form.
